I was wondering if this could ever happen? If so I would assume super duper heavy fines would ensue and I'm talking about a straight forfeit, not because a team didn't have enough players to suit up.

why would a team WANT to. ticket money, concessions. I think the answer is NO.

the rules probably allow it so I would say yes. but unless you had some real reason to do so david stern would be on the coach's ass

Anything can happen so it's possible, although incredibly unlikely. I can't imagine the circumstances where an organization would allow the coaches and players to do something like that but I can imagine the repercussions.

Players and coaches would be suspended by the league indefinitely, probably for at least a year, probably much longer. Careers would be over. Ownership would be suspended and/or attempts would be made to strip away ownership if they were held responsible for the forfeit.
